You have a 60-day special enrollment window

When you lose job-based health coverage, you qualify for a Special Enrollment Period (SEP) on the ACA Marketplace. You have 60 days from the date you lose coverage to enroll. If you miss this window, you may have to wait until Open Enrollment (November 1 – January 15). Losing coverage also qualifies your dependents separately.

COBRA: keep your old plan for up to 18 months

COBRA lets you continue your former employer's exact health plan for up to 18 months after separation. You pay the FULL premium (employer share + employee share) plus up to 2% administrative fee. The average COBRA premium is $599/month for single coverage — about 2x what you paid as an employee. You have 60 days to elect COBRA after losing coverage.

Marketplace plans are usually cheaper

After losing job coverage, compare marketplace plans against COBRA. The 2026 average after-subsidy marketplace premium is $178/month for single coverage. If your income dropped after leaving a job, you may qualify for premium tax credits that significantly reduce your cost. Use the hours-to-pay-insurance calculator to compare both options in work hours.

Medicaid is an option if income drops

If your income drops after leaving a job, you may qualify for Medicaid in states that expanded it. Medicaid has no premium and minimal copays. You can apply any time of year — there is no enrollment period for Medicaid. Income eligibility varies by state; in expansion states, it covers adults up to 138% of the federal poverty level.

Short-term coverage fills gaps

If you need coverage for a few weeks between jobs, short-term health insurance plans can provide temporary protection. These are not ACA-compliant and do not cover pre-existing conditions, but they can prevent catastrophic medical debt during a gap. Short-term plans are not available in all states.

Your prescription and doctor continuity

When switching plans, check that your prescriptions are covered and your doctors are in-network. A formulary check takes 5 minutes and can save you hundreds. Ask your current doctor for a 90-day prescription supply before coverage ends. Continuity of care rules may let you keep seeing an out-of-network doctor temporarily.

Do not go uninsured — even for one day

A single ER visit without insurance averages $2,200. A hospital stay averages $12,000+. Even one day of being uninsured is a financial risk. If you cannot afford a marketplace plan, check Medicaid eligibility. If nothing else works, a short-term plan or catastrophic plan is better than nothing. According to the Kaiser Family Foundation, 45% of U.S. adults report delaying or skipping medical care due to cost.
